Screening Questionnaires
Breast Screening Recruitment and Reminder (BSRR questionnaire form), 2001–present |
- In June 2001, to streamline mammography data collection Group Health created the Breast Screening Recruitment and Reminder (BSRR). This single questionnaire combined the in-clinic survey (IMRS) and the mailed survey (BCSP) to collect women's self-reported data.
- The goal for reformatting the questionnaire was to decrease the amount of time patients spend filling it out, and increase the completeness and accuracy of data collected for clinical care.
- In 2008, Group Health will move to a Web-based system using Health Profile, a health risk assessment (HRA). The breast cancer risk assessment questions will map to questions used in earlier questionnaires.

Interim Mammography Reporting System (IMRS questionnaire form), 1996–2001 |
- In 1996, Breast Cancer Surveillance developed the IMRS to give women the opportunity to update their risk information at their first mammogram each year. At the same time, it provided an automated recording of radiologists' interpretations of each mammogram.
- The IMRS captured assessments and radiologists’ recommendations for both asymptomatic and symptomatic women receiving mammography.
- The IMRS included a three-page women's survey and a single page completed by the radiologist with his or her findings and recommendations.
- The IMRS was more comprehensive than the data and updated risk factor information originally collected through the Breast Cancer Screening Program.

Breast Cancer Screening Program (BCSP questionnaire form), 1985–2001 |
- The Breast Cancer Screening Program (BCSP) began in 1985 at Group Health.
- Automated invitation letters were used to initiate and recall women for screening.
- A four-page self-administered questionnaire was mailed once to Group Health women, either at enrollment in Group Health or when they turned 40.
